Dear West Lothian Council,

Under FOISA, please supply me the following information held in your records:

Please advise if an emergency IRD are qualified to form a mental health analysis on an individual that would form part of any legal proceeding without having physical sight or contact with an individual?

Please advise if any member of the public without a specialised qualification in mental health are qualified to form a mental health assessment on an individual, particularly if they had not had physical sight or contact with an individual for a significant period of time?

Please advise West Lothian Council’s Social Work Emergency IRD policy and procedure on acting on unqualified members of the public reporting their personal diagnosis of an individual to Social Services?

i.e. What is the proper course of action or due diligence required to ensure the allegations or disclosures are accurate and that there is substantial risk?

Please provide your policy & correct procedure for when a qualified & substantial risk has determined?

Please provide your policy & correct procedure for when a unqualified & unsubstantial risk has determined?

Also when you have been informed by Police Scotland that there is no mental crisis?

What right of recourse or protection from such allegations does the person who is continually harassed by an unqualified individual, by reporting them to various agencies such as Police Scotland, Social Work, Medical Practitioners, to ensure that their children are removed them, and all contact with their children are removed?

Dear SocialPolicyIR,

Further to my internal review, please advise your selection criteria for for independent review.

Since 1st January 2014 until present date please advise me the number of IRD’s held year on year, month on month until present date, and alongside that show me a break down of any and all independent scrutiny of the months cases, again year on year month on month.

I am particularly concerned with the Month of August 2018, how many IRD’s resulted in a criminal arrest?

How many families/parents were separated from their children?

How many prosecutions led to a conviction?
How many prosecutions led to an acquittal ?
How many of these cases were independently reviewed?
What independent scrutiny was used? Please incluse criteria for selecting cases?
How many families have you reunited from that month?
How many families remained estranged after your Intervention?
Where there is a clear case of a wrongful decision, what is your policy and procedure to rectifying the situation?

Are you aware that the longer families are separated, the more damage is done to all concerned especially when you have got it so very wrong?

Is statement tampering and twisted assessment reports that do not represent factual accuracy the norm for covering ip your failures?

Is is this displaying professional integrity per your performance benchmarks or is concealing where you got it wrong a ‘brucie bonus’ for a social worker’s promotion?

8. Please advise the number of independent review assessors you have? The frequency of reviews? The criteria used to select cases to be reviewed?

9. If there are any conflicts of interest in independent review and scrutineers.

10. What measures can an independent review or assessor take when they come across a case where there are more than significant failures?
