Background: A briefing document issued by the NAO revealed your department has carried out secret internal process reviews (IPRs) into 69 suicides of benefit claimants since April 2014. In the seven-and-half months between 1 April 2019 and 13 November 2019, NAO states you carried out 21 IPRs into suicides linked to the benefit system, compared with 13 in the whole of 2018-19.
