Dear Sir or Madam,
I am overseeing a piece of work on specific Freedom of Information questions and ask if you could please let me know if your organization was ever the subject of an investigation by:
1) the Commission for Health Improvement (CHI)
2) the Healthcare Commission (HCC)
3) the Care Quality Commission (CQC)
Also can you tell me the number of secondments of staff at (grade 8 or above) that have taken place between your organization and the Strategic Health Authority in your area?

Thank you for your FOI request.
Before proceeding with your request, please clarify what you mean by the
term 'investigation'?
Also, as CHI were abolished before NHS South West Essex became a
statutory organisation, it is unlikely that we will be able to answer
question 1. However, I will notify you within 20 working whether or not
we can answer that question.

Thank you for your prompt response and just to clarify, any full investigations by regulators as previously mentioned.
I am aware that CHI also carried out Fast Track Reviews of Clinical Governance which I believe is just short of a full investigation but produces recommendations and action plans too.
Naturally as CHI was abolished before NHS SW Essex became a statutory organization, it may be difficult to track this information. But I am grateful for your reply and appreciate your time.

Dear D. Speers,
Thank you for your email. Sorry to cause any confusion. My email had an
attachment acknowledgement letter that is normally sent out to all FOI
requester. However, I tried to recall it as the FOI lead also sent you
an email to acknowledge your FOI request, where he also asked for more
clarification. Please ignore my acknowledgment email as the FOI lead has
acknowledge and informed that we will deal with your request in the
normal 20 working day timescale.
