I am writing to make a request for information under the Freedom of Information Act 2000.

I seek disclosure of any internal guidance, policies, or other documents that the Department for Work and Pensions (DWP) has in place concerning changes in circumstances for benefit claimants. Specifically, I am interested in:

1. Any internal guidance, policies, or documents that define what constitutes a change in circumstances for benefit claimants.

2. Any forms or procedures that claimants are required to follow if they wish to report a change in circumstances.

3. If no such form exists, please provide a list of the required questions that must be answered when a claimant calls to report a change in circumstances.
