The government is encouraging National Health Service Bodies and
Local Authorities to integrate the provision of certain Health and
Social Care services where this is appropriate.

1. Has your authority taken any steps already to integrate the
provision of any Health and Social Care services? If so, how is
this being organised?

2. Is your authority currently giving consideration to, or planning
for, the integration of Health and Social Care services in the next
1-2 years? If so, who or which department in your authority is
leading the planning for this?

I am writing in response to your request for information.

     for, the integration of Health and Social Care services in the next
     1-2 years? If so, who or which department in your authority is
     leading the planning for this?
 
 
Improving the wellbeing of Hillingdon residents by integrating health and
social care services is a key focus for Hillingdon and our NHS partners. A
number of ideas are currently under consideration and at different stages
of development. Overall, Hillingdon Council's discussions with NHS
partners are focused on an integrated approach to front line service
delivery rather than a structural integration.

 

An integrated care approach is being developed with a focus on frail
elderly people. This is examining the potential to streamline the customer
journey across health and social care provision by collaborative case
work. We are working with Hillingdon's Clinical Commissioning Group to
develop an Out of Hospital commissioning strategy focused on preventing
hospital admission. In addition, work to develop local integrated
commissioning support services for North West London is being discussed
with other West London boroughs and with NHS commissioners.

 

The Heath and Wellbeing Board provides local authority and NHS strategic
leadership for these projects. However, on a day to day basis, the Social
Services, Health and Housing Department within Hillingdon Council takes a
lead on integration. NHS Commissioners are co-located within the local
authority to aid joint approaches to our work.
