Dear Poppy and Georgia

Please accept my apologies for the delay in relation your Freedom of Information Request being completed, please be aware that this was not an intentional delay and the Trust always aims to complete information requests within the statutory timescales, where this may not be possible we will work with the requester to agree a way in which to release the information to them. I have reviewed your request and can confirm that the steps taken in relation to your request were delayed, however this was not intentional and I can see that you have received a full response to your request.

It was noted as part of the review that there was an initial delay in acknowledging your request, as our aim is to acknowledge requests within 48 hours of receiving the request. However I can confirm that the request was fully reviewed and a response drafted within three days of the acknowledgement being sent to you.

The staff members who lead on the completion of your request confirmed that they believed that the request was completed at an earlier date, which was 6th September 2019, which would have been within the statutory timescales for the request. However their error was noted when reviewing their unsent emails, the staff member identified that the email had not successfully sent and instead had saved into their Draft email inbox therefore they resent the same response to you on 18th October 2019. Our internal reporting system initially showed that your request was completed on 6th September, however that was updated to show that the final response was sent after the final 20 working day deadline, on 18th October due to the email error.

In this instance it was an error made by a staff member within my team where they expected that the email had been sent, instead this had been placed within their draft email box. I have raised the importance with staff to regularly check their draft and outbox within their emails to ensure that emails are being sent appropriately.

If you would like to take this further and formally complain to the Trust in relation to the handling of your request please contact our Patient Experience Desk [email address], information as to how to make a formal complaint is available on our website; https://www.bcpft.nhs.uk/contact-us/conc....

Regards,

Katie
