Trust Response:

Whilst the information you requested may be held by us, we have estimated
that the cost of complying with the request would exceed the appropriate
limit under the FOIA 2000.  The reasons are given below.

We do not currently record this level of information in an easily
retrievable format.  To obtain an answer would involve the Trust
completing a significant amount of data analysis, across multiple
systems.  We also believe that gathering the requested information would
place a significant burden on at least one of the departments responsible
for providing the information, as there is no capacity in the team to
gather this information without it seriously impacting on service delivery
which will ultimately have an impact on patient safety.

Please note that this response has been provided within the 20 working day
timeframe as stipulated in the Act.
