Information on Permit to Work in the NHS

I am writing to make a Freedom of Information (FOI) request to obtain information regarding the Management of Permits to work (PTW) on your sites. Could you kindly provide details on the following:
a. Which types of work require a Permit To Work to be completed on your sites?
b. Does your organisation utilise a software platform to issue, record, and provide evidence of completed works that require a permit?
c. If a software platform is used, please specify the name or type of software utilised to complete and evidence work permits.
d. Who within your organisation is responsible for the issuance, sign-off, and provision of evidence for Permits To Work?

Following receipt of your request, searches were conducted within Pennine Care NHS Foundation Trust (the Trust) to locate the requested information and I can confirm that some of the information requested is held by the Trust.
REQUEST AND RESPONSE
I am writing to make a Freedom of Information (FOI) request to obtain information regarding the Management of Permits to work (PTW) on your sites. Could you kindly provide details on the following:
a. Which types of work require a Permit To Work to be completed on your sites?
The Estates department issues a generic Contractors passport to all contractors that attend our sites to carry out work. This passport covers a number of issues around what works are to be carried out, Fire precautions, RAMs required, any permits required, Asbestos etc. The Estates team also provide Hot works permits, Fire alarm work permits, medical gas permits, Electrical LV permits etc
b. Does your organisation utilise a software platform to issue, record, and provide evidence of completed works that require a permit?
No software platform is currently used but all permits are scanned and saved electronically
c. If a software platform is used, please specify the name or type of software utilised to complete and evidence work permits.

d. Who within your organisation is responsible for the issuance, sign-off, and provision of evidence for Permits To Work?
Competent persons but usually the Maintenance supervisors or the member of the Maintenance team that is managing the contractors onsite. Some of the permits (such as Medical gas and electrical LV permits) would require an Authorising Engineer to complete.

COMPLAINT RIGHTS
Are you unhappy with how your request has been handled or do you think the decision is incorrect?
You have the right to request the Trust review their decision. However, please be advised, as per the Freedom of Information Code of Practice (2018), the Trust is not obliged to undertake a formal internal review on any complaints received 40 working days after the date of response. If a request for internal review is received after 40 working days, the Trust may not accept your request for formal review, but will take steps to assist you.
Prior to lodging a formal complaint you are welcome and encouraged to discuss the decision with the case officer that dealt with your request.
Ask to have the decision looked at again –
The quickest and easiest way to have the decision looked at again is to telephone the case officer.
That person will be able to discuss the decision, explain any issues and assist with any problems.
