Dear Freedom of Information Officer,
I am requesting the following information under the terms of the Freedom of Information Act 2000.
1. How many FOI and Subject Access requests under the Data Protection Act were received between January and May 2012?
2. How many of these requests under the respective legal category were responded to within the statutory deadline and how many missed the statutory deadline?
3. How many independent providers (GP's, Community Pharmacies, Dentists and community health providers such as Braintree Community Hospital) commissioned by Mid-Essex PCT have been independently audited against the IG Toolkit requirements for version 9?
4. How many GP's, Community Pharmacies and Dentists are there commissioned by Mid -Essex PCT? Of these independent contractors please name the contractors that completed the IG Toolkit submission for version 9 by 31st March 2012 and those that failed to complete the IG Toolkit submission.
5. What action was taken by Mid-Essex PCT or the cluster exec board against commissioned contractors for failing to complete the IG Toolkit submission as this is a contractual and mandatory requirement?

Thank you for your FOI response. I would like to know the date in which commissioned 3rd parties will be audited by Mid-Essex PCT including the new Clinical Commissioning Groups (CCGs) and Braintree Community Hospital against the IG Toolkit requirements.

I refer to your request for information, dated 06 June 2012, in which you asked the following questions.
Question 1

78 requests were received
Question 2
How many of these requests under the respective legal category were responded to within the statutory deadline and how many missed the statutory deadline?
Answer
74 were responded to within the statutory time limit and 4 missed the deadline.
Question 3
How many independent providers (GP's, Community Pharmacies, Dentists and community health providers such as Braintree Community Hospital) commissioned by Mid-Essex PCT have been independently audited against the IG Toolkit requirements for version 9?
Answer
None have been audited.
Question 4
How many GP's, Community Pharmacies and Dentists are there commissioned by Mid -Essex PCT?
Answer
159 are commissioned by Mid Essex PCT
Of these independent contractors, please name the contractors that completed the IG Toolkit submission for version 9 by 31st March 2012 and those that failed to complete the IG Toolkit submission.

Contractor status was reported to Information Governance Steering Group but no further action taken at this stage.
