Thank you for your recent request under the Freedom of Information Act
2000.

 

The information you have requested is as follows:

 

1. Have you audited your coding in the last year? – the latest completed
coding audit report was February 2019.

 

2. If so, what percentage of the records required re-coding or coding
re-validation? – The Trust do not hold details of the percentage
requiring re-coding or coding revalidation, it doesn’t quite work like
this, you get an accuracy percentage as shown below. All errors found were
fed back and amended.

3. How many e-referrals do you process - per month and year?  Please see
attached

 

4. How many ERS bookings are made - per month and year?   Please see

5. How many appointment cancellations are processed  - per month and

6. Do you have an automated process for updating General Practitioner
information changes?  Are you referring to the GP reference data (ie who
the GPs are and what Practice they are associated with) or about the GP
that the patient is associated with in the patient record?  Please see
below.

If not, how are the updates managed and what is the average delay in the
updating process?

 

If the question is about GP reference data.

There is no automated process for the Trust to manage GP/Practice set up
changes.  We have registered with the NHS Information Standards Service at
NHS digital and receive the Quarterly reference file updates.  The
relevant ones are downloaded and processed into systems as required.  The
GP related files only contain GP reference set up information – they do
not contain any information about GP/patient associations.

 

Generally the files are dealt with within a few working days of receipt. 
It has been longer with the most recent release (Nov Quarter – released
29/11/19).

 

If the question is about updating patient records with the GP.

Where patients were initially registered on PAS via the eRS we
automatically receive updated demographic information about them from the
PDS.  This can include GP data.  Otherwise demographic data is updated
manually, normally on advice from patients.

 

7. How many whole time equivalent team members process incorrectly
delivered letters? The Trust do not have designated staff members to
undertake this task within the Patient Service Centre, it is dealt with as
part of a booking clerks job role.
