Please can you provide the following information in relation to processing Individual Funding Requests for treatment:
1. Is your Individual Funding Request (IFR) service delivered in-house or is it contracted?
1a. If contracted who provides the service?
1b. What is the annual value of the contract?
1c. What is the duration of the contract?
1d. When does the existing contract expire?
2. What is the staff WTE that provide administration and management of this service?
3. What is the cost of the staff WTE?
4. When was the service last reviewed?
5. How many IFR requests did your ICB receive in 2019/2020, 2020/2021, 2021/2022?
6. Do you use a commissioning prior approval process?
6a. If yes, is service delivered in-house or is it contracted?
6b. If contracted who provides the service?
6c. What is the annual value of the contract?
6d. What is the duration of the contract?
6e. When does the existing contract expire?
7. Please provide a named lead for IFRs
