Indentifiers of current contracts for sexual health services
To whom it may concern,
I am writing to submit a Freedom of Information (FOI) request under the Freedom of Information Act 2000. This request seeks information on the identifiers of contracts currently governing the delivery of services at sexual health/genitourinary medicine clinics within CDDFT.
Specifically, I would like to request a spreadsheet file containing the following information:
1. Clinic Name: The full name of each sexual health clinic operated by or on behalf of CDDFT.
2. Contract Name/Reference Number: All unique identifiers for the contract under which services are currently delivered at each clinic. These will include any identifying names and reference numbers/IDs.
3. Operator: The group providing services at the clinic. It may be CDDFT/the NHS or a private company under contract.
