Incident reporting rates
Dear Birmingham Community Healthcare NHS Foundation Trust,
Under Freedom of Information Act 2000, can you kindly provide incident reporting rates for your wheelchair service for the years 2017, 2018 and 2019 year to date. I would be grateful if the data could be broken down as follows:
1. Reporting calendar month
2. Incident categories (i.e. falls, safeguarding, equipment failure, near miss etc)
3. Harm/severity
4. SIRI Yes or No
