It is not clear, from your email, exactly what information you require. In order for us to accurately respond to your request, we would be grateful if you could confirm the following:

• Please note that our Falls Prevention Service is situated within our Older People Adult and Community (OPAC) Services. There are no Falls Prevention Services within the other Directorates.
• As such, please could you confirm for the below questions whether you require this data for OPAC services, or for all services within the Trust?

How many falls (Per year) were recorded in the last 5 year?
How many, if any resulted in patient deaths?
How many have resulted in litigation?

The 20-day clock for response is paused while we await your clarification.

Please note that due to the volume of Freedom of Information requests we receive, we are unable to administrate further reminders for clarification.
