Incidence and survival rates of different types of cancer

To whom it may concern,

I am requesting information under the Freedom of Information Act 2000 regarding cancer survival rates and incidence.

Please provide me with a spreadsheet file containing the following data for all types of cancer on which you hold records within NHS England since 2014:
1. The median number of diagnoses per year
2. The median number of deaths caused by the type of cancer per year
3. The 1-year survival rate
4. The 2-year survival rate
5. The 5-year survival rate
6. The 10-year survival rate
7. Median age of patients at the time of diagnosis
8. Median age of patients at the time of death caused by the type of cancer
