Response to this request is long overdue. By law, under all circumstances, NHS Great Yarmouth and Waveney Clinical Commissioning Group should have responded by now (details). You can complain by requesting an internal review.

the CCG / Trust, who is responsible for them - with the best contact
details for those individuals, or if they are outsourced, who
provides those Services. In addition if the CCG / Trust provides any of
