Iatrogenic Harm

Response to this request is long overdue. By law, under all circumstances, Health and Social Care Board for Northern Ireland should have responded by now (details). You can complain by requesting an internal review.

Dear Health and Social Care Board for Northern Ireland,

1. Can you define what is your working definition of "iatrogenic Harm"?
2. Can you tell me if prescribed medication is mismanaged to cause "errors" which cause patients to suffer severe DETOX in a hospital is considered Iatrogenic Harm?
3. Can you tell me when this term "Iatrogenic Harm" appears on consultant written reports,
how it is identified and medically notated, to ensure the patient is cared for appropriately? What ICD
4. What guidelines are there for treatment of over-medication caused Iatrogenic Harm?
5. What guidelines are there for treatment of medication created health Iatrogenic Harm?
6. What guidelines are there for treatment of Iatrogenic Harm?
7. What ICD:10 WHO diagnostic codes are used to cover these Iatrogenic Harm situations.
