Can you please tell me how “iatrogenic harm” is managed within your trust?
What medical specialists are involved?
How are these services accessed?
For each of the following questions can you please provide information for each of the following years:
• In 2018
• In 2019
• In 2020
1. How many patients were diagnosed with Iatrogenic Harm.
2. What is the pathway used for managing those patients with Iatrogenic harm.
3. How many patients were treated for Iatrogenic Harm.
4. How is medication review managed in your trust?
5. How many patients in the trust use medical cannabis?
6. How many patients in the trust use medical marijuana?
7. How many patients receive ECRs to deal with iatrogenic harm caused?
8. How many patients are given ECRs to manage medication de-escalation?

Thankyou.
I deliberately try never to limit the FOI.
However I am not medically qualified, and I genuinely feel it really should not be left to me to define a medical term like "iatrogenic harm" - I feel that it should be this clarification should come from the medical community or their guidelines.
I have however a specific interest in Iatrogenic Harm caused by prescription medications and it would be helpful if that type of "iatrogenic harm" and it's management were highlighted in my request answers.

Whilst I understand your reasoning as stated above, I had clarified this comment.
I specifically wanted to know about medication prescribed which was considered to be contributing to patients' problems.
I am aware that the term exists in Patient Files and medical notes.
Can this be re-assessed as a request with this in mind?
