Dear South London and Maudsley NHS Foundation Trust,
Please could you let me know, within the next 30 days under the Freedom of Information Act:
(1) How many GP referrals to Lambeth IAPT have been (a) successful (b) unsuccessful in accessing treatment in the last twelve months?
(2) How many self-referrals to Lambeth IAPT have been (a) successful (b) unsuccessful in accessing treatment?
(3) How much does a CCG get recharged for a referral to be 'assessed'? Are there any charges in connection with the 'assessment/diagnostic'?
(4) How many 'assessments/diagnostics' did the senior member of staff in Lambeth IAPT do with patients being referred in the last twelve months? How many were (a) successful and (b) unsuccessful?
(5) How many cases did Lambeth IAPT refer onto a different part of the organisation and if so which services were referred to?
