Dear Department for Work and Pensions,
I would like clarification of how the report from a face to face assessment is created and when?

I would like you to confirm that the Work Capability Assessment is actually a benefit eligibility test,
That it is a computerised, tick box exercise and that the report is actually completed at the time of the assessment,
That there is a set format or template for these reports,
That the report form is already partially completed prior to the assessment?
