As current data from various studies show the below procedures are painful for on average 25% of women (and rising as more women are coming forward) It would be appreciated if you could answer the below questions.

1. Are consent forms always given to patients to complete prior to Hysteroscopy and/or Endometrial Biopsy procedures?
2. If not, how is informed consent obtained and data collected?
3. If the patient’s preference for any reason what-so-ever is to go under General Anaesthetic for Hysteroscopy, Biopsy or removal of polyps, fibroids or any other similar procedure and the patient is within the parameters to safely go under GA is this choice always granted?
4. If not do you hold data with refusal reasons and the name of the professional who has refused?
5. If data is held on reasons for refusal could you forward that data on for the last 12 months?
