09/06/2016 - Balance Sheet - expense type 'Gym' - Incorpore Limited - ref SIV 131077 - value £ 8,367.00 - annual membership

01/03/2017 - Gym, Employee Rewards - expense type 'not assigned' - Incorpore Limited - ref SIV-147266 - value £2.083.00 - annual gym membership 01/03/2017

24/04/2017 - Gym, Employee Rewards - expense type 'not assigned' - Incorpore Limited - ref SIV-149841 - value £1,481.00

02/05/2017 - Not assigned - expense type 'Gym - Employee Rewards' - Incorpore Limited - ref SIV-150258 - value £2,083.00

22/08/2017 - CEO Directorate - expense type 'Gym' (2), not assigned (1) - Incorpore Limited - ref SIV 157054 - value £886.00 x 3 transactions

19/12/2017 - Gym - expense type 'not assigned' - Incorpore Limited - ref SIV 163963 - value £1,368.00

19/12/2017 - Gym - expense type 'not assigned' - Incorpore Limited - ref SIV 163994 - value £3,508.00

02/05/2018 - 5 separate transactions - 'Gym' and 'Employee Rewards' - Incorpore Limited - refs SIV 170986, 170987, 169315, 170988, 170989 - total value £ 19,602.00

If the beneficiaries of any of these transactions were not in the employment of HS2, please confirm the name of the beneficiary, the nature of their connection with HS2 Limited and their employer.
