How many WCA/PIP assessors have been reprimanded/sacked for poor quality reports?

Response to this request is long overdue. By law, under all circumstances, Department for Work and Pensions should have responded by now (details). You can complain by requesting an internal review.

Dear Department for Work and Pensions,
How many PIP/ESA assessors have been reprimanded/sacked due to providing poor quality reports where appeal has overturned original DWP decision?
Similarly how many times have Atos/Maximus/Capita been fined/sanctioned/reprimanded by DWP for poor quality reports resulting in a win at appeal?
Similarly how many DWP decision makers been reprimanded/sacked for making the wrong decision on a claim that the courts have overturned?
