How does the Home Office deal with reports of BRPs that are not received?

When a BRP is reported as not received after 10 working days via your online form, what process is followed to determine the reasons for the delay?
If an issue cannot be resolved immediately, what response is given to the customer, and after how long is the issue reviewed?
What are the most common causes recorded for the delays in the issue of the BRPs?
