Dear Derbyshire County Council,
With reference to a DCC Public Health Covid-19 Emergency Fund grant of €960 awarded to the Chesterfield based community group Holme Hall Unite dated between 06/10/2020 and 21/12/2020, under the Freedom of Information Act I am requesting electronic or hard copies (as appropriate) of the following documentation :-

• The completed Grant Application Form.
• The DCC response to the Grant Application Form.
• Any evidence in DCC's possession that confirms or otherwise that the funding has been used by Holme Hall Unite for the purpose it was awarded.
