Dear Sir or Madam,

HM Coroner Mr Tom Osborne & The Case of Mr Stephen Carré

Please can you supply appropriately redacted copies of ALL documents you have in relating to the case of Mr Stephen Carré who died in January 30th March 2010, and for which the coroner, Mr Osborne provided what I believe to be a "Rule 43" letter/report issued 30th March 2010 (or thereabouts) regarding the 'prevention of future deaths'?

Please can you supply copies of ALL documents, faxes, emails, minutes, court transcripts and contemporaneous notes of telephone calls. Of material importance is the reply/ correspondence from (and to) the DWP and/or government between March 2010 and 31st July 2016?

This death is not a Milton Keynes death and so we have no knowledge of any inquest or subsequent Rule 43's.

During 2010 Mr Osborne was also active representing Bedford, Northampton and Gloucestershire Coroner's areas.
