Histopathology Inquiry - Paediatric Pathology Misdiagnoses

Waiting for an internal review by University Hospitals Bristol NHS Foundation Trust of their handling of this request.

Dear University Hospitals Bristol NHS Foundation Trust,

Please advise whether the families affected by the Paediatric Pathology misdiagnoses that occurred from 2001 to 2008 inclusive have been informed of the misdiagnoses and told that the problem was included in the Bristol Histopathology Inquiry commissioned by the Trust.

If the families were informed, please confirm when and how they were informed.

If they were not informed, please explain the reason.

Yours faithfully,

Mrs D Havercroft

Mrs D Havercroft

Dear University Hospitals Bristol NHS Foundation Trust,

Please pass this on to the person who conducts Freedom of Information reviews.

I am writing to request an internal review of University Hospitals Bristol NHS Foundation Trust's handling of my FOI request 'Histopathology Inquiry - Paediatric Pathology Misdiagnoses'.

I quote from Mrs Brunt's statement on your website:

North Bristol NHS Trust statement on the recommendations of the Histopathology inquiry
I accept on behalf of North Bristol NHS Trust the findings and recommendations of the independent Histopathology inquiry and we will be acting on them accordingly.

With new senior management in place at both Trusts we now have a better working partnership between both organisations.

With University Hospitals Bristol NHS Foundation Trust (UH Bristol) we have agreed to appoint a Joint Clinical Director of Histopathology, to cover both Trusts, who will be based at North Bristol NHS Trust. Also, we have already put in place an agreed escalation protocol for clinicians to raise their concerns which will ensure rigorous and swift investigation.

Ourselves and UH Bristol have contacted the individuals or their relatives of all the cases who were scrutinised as part of this inquiry, to let them know why their case had been reviewed and to provide them with proper advice, reassurance and guidance.
Moving forward we are committed to an effective and supportive working partnership with UH Bristol and to continue providing good quality and safe healthcare for our patients.
Mrs Ruth Brunt
Chief Executive
North Bristol NHS Trust
8 December 2010

Mrs Brunt says that NBT and UHBT jointly contacted all the people/relatives affected by the cases that were reviewed in the Inquiry - ie. the 26 adult cases.

Therefore it is a matter of public interest to know whether the children/families affected by the paediatric misdiagnoses were treated equitably and also informed about the Inquiry and provided with "proper reassurance, advice and guidance"

Please answer my FOI question.

A full history of my FOI request and all correspondence is available on the Internet at this address:

Yours faithfully,

Mrs D Havercroft
