Good morning.

I wish to gather information regarding NHS Health Checks in your area.

1. What is your approach to improving Health Check performance?
2. What is your annual budget for Health Checks?
3. How much do you pay service providers per Health Check?
4. Are Health Checks performed in the community, or purely via GP Practices? (e.g. health kiosks, occupational health, privately, etc.)
5. Do you have certain GP Practices who do particularly well, which may skew the average on the PHOF website, or do they generally all perform well?

Kind regards,

Mr B. Akehurst

Dear Mr B Akehurst,

Freedom of Information Act 2000 - Request for Information

I am writing in respect of your recent enquiry for information held by the
Authority under the provisions of the Freedom of Information Act.

Please find the response to your request below.

1.    What is your approach to improving Health Check performance?

We have commissioned a lead provider model for the delivery of the NHS
Health Check which includes a targeted approach for those individuals who
have a BMI over 25 and/or who smoke.

2.    What is your annual budget for Health Checks?

The annual budget is £150,000 per annum

3.    How much do you pay service providers per Health Check?

Our Provider, Conexus Healthcare Ltd, determines the cost per health
check.

4.    Are Health Checks performed in the community, or purely via GP
Practices? (e.g. health kiosks, occupational health, privately, etc.)

The majority of NHS Health Checks are completed in GP Practices but our
Provider has just implemented the provision of the NHS Health Check
service in extended hours, weekends & evenings.

5.    Do you have certain GP Practices who do particularly well, which may
skew the average on the PHOF website, or do they generally all perform
well?”
