Health Checks in Stockton-on-Tees

I wish to gather information regarding NHS Health Checks in your area.

1. What is your approach to improving Health Check performance?
2. What is your annual budget for Health Checks?
3. How much do you pay service providers per Health Check?
4. Are Health Checks performed in the community, or purely via GP Practices? (e.g. health kiosks, occupational health, privately, etc.)
5. Do you have certain GP Practices who do particularly well, which may skew the average on the PHOF website, or do they generally all perform well?

Thank you for your correspondence received on 12 June 2019. In response to
your request I can now provide the following information. 

Q1. What is your approach to improving Health Check performance?

A1. SBC improves HC performance by

o Coordinating free training to staff delivering Health Checks
o Provision of resources (leaflets, results booklets)
o Quarterly newsletter to practices/providers
o Data support for providers e.g. invitation lists, templates for data
input and feedback on performance
o Performance related incentives
o Awareness campaign  
o  

Q2. What is your annual budget for Health Checks?

A2. £ 154,000

 

Q3. How much do you pay service providers per Health Check?

A3. SBC pays service providers

o £2.50 per first invitation
o £16 per completed Health Check

+ £10 if annual performance target of 60% achieved

+ £10 if patient has risk score > 20% and has been offered medication
and/or referred to lifestyle services

+ £10 if patient is from most deprived community (national quintile 1)

Q4. Are Health Checks performed in the community, or purely via GP
Practices? (e.g. health kiosks, occupational health, privately, etc.)

A4. GP practices only

Q5. Do you have certain GP Practices who do particularly well, which may
skew the average on the PHOF website, or do they generally all perform
well?

A5. All GP practices in Stockton are signed up to deliver the NHS Health
Check.

The performance of practices varies considerably with outliers in both
under- and over performance. The impact on the average on the PHOF website
by particularly well performing practices is therefore balanced by those
practices which perform less well.
