Dear B Akehurst,

Freedom Of Information Act 2000 - Information Request 20518

Your request for information received on 14/05/2019 has now been
considered and the information requested is enclosed.

Your Request and Our Response

What is your annual budget for Health Checks?
For 2017-2018 It was £491K. An incentive scheme funded by reserves
increased the spend to £779K

How much do you pay service providers for Health Checks, per Health Check?
£25 per GP health check and £20 per check completed by pharmacies and
optical practices.

What software do you and/or the GPs in Dudley use to monitor Health Check
activity?
EMIS Web with central search and report software.

Are Health Checks performed in the community, or purely via GP Practices?
(e.g. health kiosks, occupational health, privately, etc.) What is your
approach to improving Health Check performance?
Health checks are completed in:

* GP practices
* Pharmacies
* Optical Practices
* Local Lifestyle Services provider
* 3rd Party provider

Working with our Clinical Commissioning Group, as part of the Vanguard, we
have developed a local alternative to the Quality and Outcomes Framework
(QOF) in the National GP Contract. The Dudley Quality Outcomes for Health
has replaced the QOF indicators and as part of its Public Health Domain,
it has performance indicators on invitation and completion of NHS Health
Checks.

As we had a slower than expected start to the 5 year cycle due to the
changeover of practices to EMIS web, we decided to introduce an incentive
scheme for practices to catch up in the final year offering a bonus
payment to practices who attained 66% health check coverage of the 5 year
eligible cohort, with a second incentive tier at 75% coverage.
