Can you provide the following information
1. The form they have to fill in with the questions they are expected to answer.
2. How many staff by department are required to have these health checks.
3. By department what are the grades of the staff who have to have these health checks.
4. Across the authority how many Grade 10 and above staff are required to have these health checks. Are there any special arrangements for Chief officers?
5. Do these health checks require staff to disclose any or all of the following do they
a) smoke
b) use drugs
c) drink
d) gamble
e) obese bmi
If staff decline to answer what are the repercussions, is this in a policy or procedure manual?
6. Please provide the policy and protocols as applied to health checks
7. Please provide the costs of these health checks over each if the last two financial years
8. Who is contracted to provide these checks

I understand that some but not all employees are required to undergo
regular (annual or otherwise) health checks at the expense of the tax
payer.

 

In response to your request the answers below have been provided based on
health checks undertaken for health surveillance purposes, which may be
required by law for employees who are exposed to certain hazards due to
the nature of their work e.g. vibration/ noise/ hazardous substances.

 

Public Health also provide the national NHS health checks for 40 to 74
year olds, across the city, but this is for the eligible population of
York through the Health Trainer Service.

 

Can you provide the following information

1.    The form they have to fill in with the questions they are expected

One.

There are no special arrangements for chief officers, if their role
requires health surveillance then they will be required to attend

 

 

5. Do these health checks require staff to disclose any or all of the

d) gamble

e) obese bmi

 

Please see attached questionnaire that is completed prior to appointment
with either an occupational health advisor or technician

 

If staff decline to answer what are the repercussions, is this in a policy
or procedure manual?

 

The requirement for Health Surveillance is linked to potential hazards in
the workplace. As the matter pertains to an individual’s health the usual
legislation around access to medical records apply. We would however view
the requirement to attend as a reasonable management instruction and
failure to participate may result in the Council taking whatever action is
felt appropriate in the circumstances

 

6. Please provide the policy and protocols as applied to health checks

 

A copy of the health surveillance form can be found in response to
question 1 above, with the actual health surveillance carried out by our
occupational health provider.  The requirement for health surveillance is
covered by the Council’s Health and Safety Management System through its
compliance notes. An example is included. 

 

 

7. Please provide the costs of these health checks over each if the last
two financial years

 

We are unable to provide this figure as it forms part of the cost of the
overall Occupational Health Service. On an annual basis the total budget
for the OH contract equates to £109k.

 

8. Who is contracted to provide these checks

 

We have just awarded the contract for Occupational Health to Health
Management Limited.  Health Surveillance is included within the contract.
