This request relates to the Health Board’s PTR investigations.

Delineated by financial year, I’d be grateful for you to provide figures from the last 5 years on the number of:

a) PTR investigations which have been investigated either in relation to an allegation of harm highlighted in the concern or to identify a potential incident that has caused harm;

b) PTR investigations the Health Board has investigated for the potential existence of Qualifying Liability;

c) PTR investigations where a Qualifying Liability has been established.
