Health and Safety Fines and breaches

I am writing to submit a Freedom of Information request regarding any fines that may have received due to breaches of health and safety regulations in the last 5 years.
Specifically, I would like to obtain information about instances where the council has been fined for such breaches.

Please provide the following details for each instance, if applicable:

Date of the fine
Nature of the breach
Amount of the fine
Any actions taken by the council in response to the breach and subsequent fine

ANS The commission to WSP was to provide technical support to Oldham
Council. This included specialist advice on the management of Bridge
joints and structural assessment of Bridge joints.

refurbishment works to Manchester Street Viaduct. This included specialist
