Dear Oxfordshire County Council,

Following the inquest into the death of Jennifer Wong at the Headley Way / London Road Junction. Oxfordshire County Council received HMSC Salter’s Report to
Prevent Future Deaths made under Regulation 28 of the Coroners (Investigations)
Regulations 2013 dated 2nd September 2022 (The Report). The response to The Report was: https://www.judiciary.uk/wp-content/uplo....

The Report noted the following:
1) Stage 4 Road Safety Audit (RSA) – The RSA4 was completed in October
2022.

Please will you provide me with a copy of this report: RSA4 for the Headley Way / London Road Junction.

2) Vulnerable Road Users (VRU) Audit – following a request from the family, this
was commissioned though our Highway maintenance provider on Friday 7
October 2022. ... The council will write to the coroner with the
recommendations of this report upon receipt.

Please will you provide me with both the VRU Audit for this junction and the resulting correspondence with the Coroner giving the recommendations of this report.
