Further to your email requesting information under the FOI Act
2000, please find below the Trust's response. Please accept our apologies
in the delay in providing you with a response.

"Please give details of any reported security issues relating to the
secure storage lockers situated in the Reception of Hatherton Centre,
Stafford" and

ONE: Regarding the above lockers, how many breached security events, if
any, were reported this year?

TWO: What manner of problem(s), if any, were reported? For example:
alleged pilfering from the boxes, or alleged sharing of locker keys, or
evidence of duplicate keys with nefarious motives ... (these are purely
examples and not meant to be accusatory).

I can report that during 2010, a visitor highlighted an issue whereby he
had been provided a key to a locker by the reception staff only to find
that it was already in use, I believe that this may have occurred on two
separate occasions. As a result the procedure for the storage and
allocation of locker keys was reviewed and amended.

It has not been our experience that these lockers have been used for any
illegal purpose or in any way which might deliberately subvert security
in the Hatherton Centre.

THREE: What role, if any, did the Hatherton Centre's CCTV live, digital
quality, real-time surveillance help to track the source of the
problem(s)?

With regard to the problem highlighted with the lockers, we were able to
identify the cause of the problem without the need to review CCTV
footage.

FOUR: Who is the most senior person of your Trust responsible for the
security of these lockers at Hatherton Centre, Stafford?

With regard to the day to day operation of the security lockers in the
Hatherton Centre I am responsible for security supported by a security
coordinator, however ultimate responsibility rests with Sue Havers,
Clinical Director

FIVE: Who is the supplier to your Trust of these lockers and when were
these lockers installed and whose job is it to maintain these lockers?

With regard to the supply and maintenance of the lockers, they were
purchased generically and do not require any maintenance other than
weekly searching which takes account of any potential defects.

SIX: Is there, within the reception of Hatherton Centre, Stafford, any
kind of written notice related to the use of these lockers, something
that might read: 'At The User's Own Risk' or similar?

Currently there aren't any statements to the effect that lockers are
used at the individuals own risk, however in view of this suggestion it
will be taken to the directorates security and risk committee for
consideration.

SEVEN: Please confirm that these lockers are used regularly and equally
by: (a) Service users / patients (b) members of staff (c) visitors,
(either private or professional), (d) visiting workmen, and so on.

I can confirm that the lockers are indeed provided for the use of
service users, visitors and members of staff and are frequently used by
members of all three groups
