The Department of Health published the document 'Best Practice in Managing Risk - Principles and Evidence for the Best Practice in the Assessment and Management of Risk to Self and Others in Mental Health Services'; within this document it describes the tools for supporting best practice, the questions that I have asked would pertain to information held by the Department of Health.
