Dear Department for Work and Pensions,

Please send me a copy of the forms used to apply for Hardship
payments for people with a benefit sanction and the guidance
given to staff and decisions makers to assess eligibility and
copies of all of the standard letters issued throughout the process
of applying for these payments, from initial application, being
awarded payments, being denied payments and rights of appeal.

Together with copies of information that is given to people when
they are notified their benefits have been sanctioned on how they
can apply for hardship payments.
