This is a reminder that I have requested a copy of the Hardship Payments (HP) application form for Universal Credit (UC). If the applicant does not complete any form, please provide a copy of any questions or assessments guidance information that the DWP use to assess whether to award HP under UC or a print out of any specimen documents that have to be signed a part of a UC HP claim, by email please.
