Dear Liverpool University Hospitals NHS Foundation Trust,

I request the below information under the provisions of FOI 2000. I am led to believe that certain Trust staff are required to now be issued with “Smart Cards” for certain IT access. In order to obtain this “smart card”, the staff member must produce multiple copies of identification.

- How many staff across the Trust currently have smart cards allocated?
- How many staff are due to be issued a smart card?
- What forms of identification are required or are considered to be acceptable forms of identification before the smart card is issued?
- Which department / team are responsible for handling these forms of identification when produced by an employee for a smart card to he issued?
- How is the copies of this identification subsequently stored? Electronically or hard copies?
- What is the retention period for the copies of the identification to be stored?
- How many employees have access to the copies of the identification?
- A copy of any policy documents relating to the rationale for why identification is required when a smart card is allocated
