Dear Oxford University Hospitals NHS Trust,

Please can you supply the following information, this information was asked to be stored by the government of the time.

a) A list of companies from which Factor VIII (Hemophilia Treatment) was purchased from during the period of 1980 - 1986 at Oxford Haemophilia Centre.

b) A list of batch numbers of Factor VIII used at and provided by Oxford Haemophilia Centre during 1980 - 1986 and their manufacturers.

I am writing to respond to your request sent on the 16^th April 2016.
Oxford University Hospitals NHS Foundation Trust  can confirm that it
holds the data that you requested.

As per discussion the Trust will not be able to release the manufactures
of the listed products. With your approval this information has been
omitted.

Please note that on 1 November 2011 the Oxford Radcliffe Hospitals NHS
Trust (ORH) merged with the Nuffield Orthopaedic Hospital NHS Trust (NOC)
to form the Oxford University Hospitals NHS Trust (OUH). Our response
reflects these changes. Therefore, we consider that Oxford University
Hospitals Trust has released to you all of the information that it holds
in relation to your request.
