Please could you do this in the format [Practice Number, Practice Name,
Practice Address, Branch code (if available))”

 

 

We do hold information on the branch sites of surgeries and we are pleased
to be able to provide this information in the attached spreadsheet. Please
note that we use ‘w-codes’ to identify individual GP surgeries and their
branches. The specific branch site code is contained within the practice
name field (as held), with the main site code being signified in the final
column.
