Dear NHS Crawley Clinical Commissioning Group,

Please can you provide me with a list of GP surgeries which have applied for closed list status in the last year.

Please indicate whether these applications were granted.

If granted please indicate the date when the list with closed and the period for which it was closed.

Please also provide me a list of surgeries with lists that are currently "capped".

Please provide me with the total number of GP surgeries that you commission.

In normal circumstances we should be able to (a) confirm or deny that we
