REQUEST FOR INFORMATION UNDER THE FREEDOM OF INFORMATION ACT

Further to your request for information, please find attached 3
documents in relation to your request. Apologies for the delay in
getting this to you.

The first reference was disseminated in a prescribing newsletter
(September 2002). This has been periodically updated and reissued.

Work on reducing usage was introduced as part of the 2007/2008
prescribing changes when the PCT explicitly stated that non insulin
dependant Type 2 diabetes patients should have test strips removed from
repeat prescriptions and instead given out at annual reviews or in
response to a specific request given in response to a definite need (see
attached Audits PBC incentive schem1.doc)

There is also reference to this policy as part of the 2008/2009 PIS (see
attached PIS blood Glu.doc)
