I would therefore appreciate if the requested information could now be provided.
I reserve the right to bring this to the attention of the Information Commissioners' Office (ICO), should I not receive a response to my request.
Kind Regards,
Luke Wood

Dear Joanne,
Thank you for providing this information.
Can you confirm if your trust does not collect diagnosis data in general for outpatients, or whether this is specific to not recording Functional Neurological Disorder diagnoses?
Kind Regards,
Luke Wood
Good morning,
Thank you for getting in touch about this, and I am sorry to hear about
the delays you experienced.
The response to your request has now been provided so while I apologise
again for the length of time taken to respond, this may now have removed
the need for an internal review. If this is not correct and you would
still like to pursue an internal review, please let me know and if
applicable provide a little more information about any additional concerns
you have.
I am aware that you have asked a brief follow-up question in relation to
the response provided, which my colleague is looking into and we should be
able to answer within the next couple of days.
Best wishes
