Dear FOI Team,

I would be most grateful if you could provide me, under the Freedom of Information Act, details in respect to your contract for Friends and Family Test Surveys.

1.Are you using a 3rd party providers platform to manage the FFT surveys, please provide the name?

2.Actual spend on this contract/framework (and any sub lots), from the Start of the contract to the current date with costs per annum.

3.Original Start date of the contract

4. Expiry date of the original contract ?

5. Is there an extension clause in the framework(s)/contract(s) and, if so, what are the duration of the extensions and what stage of extensions are you currently on ?

6. Has a decision been made yet on whether the framework(s)/contract(s) are being either extended or renewed and to what period?

7. The pay bands of staff and Whole Time Equivalents directly responsible for the management of the Friends and Family Test.

8. Please advise on all the methods of collecting feedback used : ( For example, online survey,telephone survey, sms, paper forms).

Acknowledgement of Request for information under the Freedom of
Information Act 2000

Thank you for your request for information, made under the Freedom of
Information Act 2000 (the Act) which was received by Portsmouth Hospitals
University NHS Trust.

Under the terms of the Act the Trust expect to respond to your request
within 20 working days. However, the Trust will also investigate the
nature of the information requested. If clarification is required in order
to identify the information requested then the response period will be
suspended until such time as the FOI Team receive clarification.

Please note that in accordance with section 12(4) of the Act where
multiple requests for information are received from one person or by
different persons who appear to be acting together in a single campaign,
the estimated cost of complying with any of the requests is to be taken to
be the estimated total cost of complying with all of them. Where
applicable, we may deal with multiple requests for information under a
single reference.

There are also a number of exemptions which the Act permits with respect
to disclosure of information. The information will be assessed for these
exemptions prior to releasing the information to you. The FOI Team will
advise you if we are unable to provide the information requested due to
exemption(s).

The supply of information under Freedom of Information is intended to be
