Dear FOI Team,
I am writing to request the following information under the Freedom of Information Act 2000:
1. WHO Pre-Operative Checklist Compliance:
Please disclose Rochdale Hospital’s Obstetrics and Gynaecology service’s yearly compliance rates with the World Health Organisation’s (WHO) pre-operative checklist for the years 2020, 2021, 2022, and 2023.
2. Executive Attendance at Departmental Meetings:
From which date were Rochdale Hospital’s Executive Managers (and from 2021 onwards, the Northern Care Alliance’s Executive Managers) invited to attend front-line Monday morning departmental meetings within the Obstetrics and Gynaecology service?
How many invitations did Rochdale Hospital’s and the Northern Care Alliance’s Executive Boards receive requesting their attendance at the Obstetrics and Gynaecology service’s front-line Monday morning departmental meetings?
3. Job Descriptions and Learning from Executive Involvement:
Please provide the job descriptions of the individuals who fulfilled the roles referenced in Question 2.
Disclose any subsequent learning outcomes or initiatives shared across Rochdale Hospital and the Northern Care Alliance from 2021 onwards as a result of their involvement.
4. Overrides of Consultant Decisions Due to Bed Shortages:
How many episodes were recorded where Nurse Managers (ADNS) or Executive staff attempted to override Consultant decisions to admit emergency obstetric cases into Rochdale Hospital due to a lack of staffed beds?
5. Guidance and Resources to Mitigate Bed Shortages:
Given the lack of staffed beds across the Obstetrics and Gynaecology Department, what specific guidance and expanded resources were provided by Executives and Nurse Managers (ADNS) to front-line staff to ensure patient safety was not compromised?
