I am writing to request information under the Freedom of Information Act 2000 regarding patient safety procedures and trust investigations, as they pertain to recent developments highlighted by Revd Graham James, chair of the Paterson Inquiry.
1. Please provide a copy of the national patient recall framework issued by NHS England in June 2022, fulfilling the recommendation of the Paterson Inquiry.
2. Could you provide details on how patient safety is prioritized and ensured in all recall procedures outlined in the aforementioned framework?
3. In reference to concerns raised by Revd Graham James regarding the thoroughness of patient recalls, particularly in the case of Williamson's patients, I request information on any independent reviews conducted by NHS England to assess the recall procedures, especially when thoroughness has been questioned.
4. Regarding the recent recommendation made by Revd Graham James for NHS England to intervene in trust safety investigations, could you provide any documentation or correspondence related to this recommendation, including any responses or actions taken by NHS England in light of this recommendation?
5. Please provide any reports, summaries, or findings resulting from the initial stage of the review conducted by the Northern Care Alliance regarding patient recalls.
6. Additionally, I would like to request any correspondence, memos, or documents related to Revd Graham James, specifically any communications with NHS England regarding patient safety procedures or trust investigations.
7. Furthermore, please include any documents or communications related to Revd Graham James's recommendation for NHS England to intervene in trust safety investigations, as reported in the media.

This is a wide angled search and would involve contacting most managers
and clinicians for any data they held. It would help if you could limit it
down to a department or individual as this at present would take too long
to attempt to find out.

related to patient safety procedures, reviews, or decisions made by NHS
England or its affiliated healthcare providers, particularly those
relevant to ensuring patient safety.

There is a reference to the FOI being long overdue, but this was responded to on 31st May 24. The request contained 8 questions. We have either pointed towards NHS England or stated we do not hold. The NCA hold no further data, and the response was not 'long overdue'.
