Would you please release:
1. The criteria for acceptance as a member for active lifestyle scheme as applied to those referred by the GP and Cardiologists.

2. The number of GPs and Cardiologists referrals and applications for membership, and the numbers of accepted and refused applications, for the year 2017 to-date in 2018.

3. The number of applications (a) accepted and (b) refused which relate to those who are currently partaking in regular physical activity with the Leicester City Council and are members of the Leicester sports centres, and respect of the refusals, the criteria/rules applied to justify the refusal and the period before a new application for the active life scheme/the six months free scheme will be considered.

4. Information held by the council relating to the standards of equipment required to be provided in relation to the scheme; and any information related to breaches of these requirements in the current year, to-date.

5. Information on any complaints, or accident reports, relating to equipment provided in relation to the scheme received in the current year to-date"

6. For the Spence Street sports and Evington Leisure Centres, please provide the copies of OFSTED reports for the year from 2016 to 2018.

7. The cost of refurbishment including installing new equipment/fitness suite, two pools and spectator seating for 400 people at the Braunstone sports Centre

8. I would also like to request the dates of any official visits by the mayor to the Spence Street sports centre and Evington Leisure Sports centre including copies of the minutes of those meetings.

9. If the meetings are not minuted then please provide copies of the minutes, notes action points/plans concerns etc that has been recorded.

Many thanks for your detailed response to my Freedom of Information request,
I realise some elements of this correspondence are seeking clarification and others amount to requests for information not included in my original request. I hope my Leicester City Council would not hesitate to deal with this in a sensible and polite manner, which results in the information I am seeking being released. I am also of the opinion that the subject matter is close enough to include in this correspondence thread.
In respect of your answer to my first question, could you please clarify who carries out the Active Lifestyle Scheme prioritisation and assigns individuals to the priority groups?
In respect of your answer to my question 3 could you please breakdown the number of refusals for each of the criteria given and release the length of any waiting period for making new application following a refusal.
In respect of your answer to question 7, recently the Braunstone Leisure centre has undergone fully refurbishment. Could you please provide the date of completion of the refurbishment and information of re-opening? If it is not yet completed when will it be complete? The income generated from this sport centre for the financial years 2016 to 2018.
Could you also provide the income generated from the Spence street sports centre and Evington Leisure Centre for the financial years 2016 to 2018?
The refurbishing of Spence Street and Evington Leisure Centres when it will start? How would Leicester City Council involve the local communities in making refurbishment decision for these centres?
