Apologies for the delayed response.
We have commissioned three reviews-
Diabetes Service- 2019 this is attached.
Trauma & Orthopaedic- 2021 this is attached.
Please note for the third review the release has been refused as it is subject to the Section 41 exemption - information provided in confidence as part of an investigation. This is an Absolute Exemption, therefore the Trust in this case must refuse to release the report.
The Section 41 exemption applies to information that has been obtained from or about other people in circumstances that identify the individuals as part of an external confidential investigation and the report also contains information provided confidence.
The Investigation does identify Trust staff and could also lead to indirect identification of patients.
The Trust takes its duty to protect personal confidential information seriously and believes there is a wider public interest and expectation that healthcare organisations do not release information collected in relation to Royal College reviews lightly. The Trust believes that release of this information could cause potential distress to the staff and patients
