Dear Department of Health,
I am writing to make a request for all the following information which I am entitled to under the Freedom of Information Act 2000.

Please provide a copy of the following from the Department of Health & Health and Social Care Board related to foster care commissioned services.

Please provide details of the spending broken down by financial year for each of the last 5 years.
1) What was the spending on the Fostering Attainment and Achievement programme?
2) What was the spending on commissioned foster carer ‘independent’ support services (please break this down if this includes separate commissioned service for allegations support and Fostering Network membership.)

3) Please provide copies of the most recent evaluations of these services.
4) Please provide copies of the Department of Health & Health and Social Board’s documented involvement of foster carers in the planning, delivery and evaluation (commissioning) of the Fostering Attainment and Achievement programme (as required by The Health and Social Care Reform Act (NI) 2009).
5) Please provide copies of the Department of Health & Health and Social Board’s documented involvement of foster carers in the planning, delivery and evaluation (commissioning) of the foster carer ‘independent’ support services (as required The Health and Social Care Reform Act (NI) 2009).

I would like the above information to be provided to me as an electronic copy.
If the request is too broad or unclear, I would be grateful if you could contact me as I understand under the Act you are required to advise and assist requesters. If any of the information is already in the public domain, please send it to me directly with page references or URLs when necessary.

If it is not possible to provide the information requested due to the information exceeding the cost of compliance limits identified in Section 12, please provide advice and assistance under the section 16 obligations of the Act, as to how I can refine the request.
